As you engage with our programs, holistic wellness, career coaching, and/or the PSYCH-K® method, here are some important details to keep in mind.
Medical Disclaimer

It is important to know that Holistic Wellness Coaching and the PSYCH-K® method are not a substitute for professional medical treatment, rather they are often a complement to it.
The PSYCH-K® method is strictly limited to the modification of beliefs. The PSYCH-K® method is not designed to diagnose medical problems, nor is it a replacement for medical attention or professional medical care and treatment.
Holistic Wellness Coaching programs, including but not limited to the Emotional Resilience 101 Program, are not designed to diagnose or prescribe treatments for mental health issues or mental disorders such as clinical depression, anxiety disorders, bipolar disorders etc.
Client’s Responsibility

By engaging in a paid session or enrolling in our program, Client understands and agrees that she/he/they are individually responsible for her/his/their own life and its unfolding.
Further, Client agrees that as an expression of her/his/their responsibility she/he/they is seeking assistance with the beliefs that manifest her/his/their reality and/or with learning and practicing approaches to managing anxiety and stress. Client further understands that the actual manifestation of that reality and/or the effectiveness of their ability to manage anxiety and stress is up to her/him/them.
Client Consent
Except in the case of gross negligence or malpractice, you or your representative(s) agree to fully release and hold harmless Susan Uramoto, dba Beliefs Transformed, from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s) or services provided.
